What Is a Standalone ALIF?
Anterior lumbar interbody fusion (ALIF) is a surgical procedure performed through the front of the body to fuse vertebrae in the lower spine. In select patients, a “standalone” ALIF — meaning no additional surgery from the back of the spine is needed — can be an excellent, less invasive option for relieving chronic low back pain.
While some patients require a combined approach (surgery from both the front and the back), others are ideal candidates for the standalone procedure.
A Patient Success Story
The imaging shown here represents a real patient who proved to be an excellent candidate for a standalone ALIF. He presented with single-level severe disc collapse and degeneration at L5-S1 — the junction between the lowest lumbar vertebra and the sacrum.
After surgery, he achieved significant relief from his back pain and returned to normal activity relatively quickly. Cases like his illustrate how powerful the right procedure can be when matched to the right patient.
Key Factors for Standalone ALIF Candidacy
Not everyone is a candidate for this approach. Two of the most important factors are good bone quality and degeneration isolated to a single disc level. When both are present, outcomes are often excellent.
Questions to Ask Your Surgeon
If you’re considering whether a standalone ALIF may be right for you, here are important questions to bring to your consultation:
- Do you smoke? (Smoking significantly impacts bone healing and surgical outcomes.)
- Do you have any medical conditions that affect bone quality, such as osteoporosis?
- Are you in good physical condition with strong core, back, and abdominal muscles?
- What activities are most important for you to return to after surgery?
- Are your facet joints — the small stabilizing joints at the back of the spine — in good condition?
